Anonymous Report of Discrimination
This form is for reporting discriminatory behavior or incidents of discrimination to the Working Group on Equal Treatment Issues (short: AKGl). Either you were personally affected or witnessed such an incident. By submitting this report, you help us gain a better understanding and address discrimination at the MU
You can use this form to report discrimination based on gender, ethnicity, religion or belief, age, sexual orientation, as well as sexual harassment.
Reports regarding discrimination basend on disability can be submitted to the Disability Liaison Officer.

- Fill out the form
- Place the printed form in a plain white envelope and seal it. Address the envelope to the AKGl to prevent it from being opened unintentionally by unauthorized persons.
- Please drop the letter in the AKGl mailbox. It is located on the building at Schöpfstraße 45, next to the entrance door.
How to stay as anonymous as possible
- Fill out the form on a computer to avoid any identification based on your handwriting.
- Write everything in lowercase; avoid typical phrases, expressions, or abbreviations that could lead to your identification.
Anonymous reports are documented and included in the statistics but are not investigated further. If you would like to take active steps to address an incident, please feel free to contact us in person, by phone, or by email. All consultations are confidential, and no action will be taken without your consent.
Duty of truthfulness! Please note that your report must be true based on the information available to you at the time of reporting.
